Getting Started
Following is a list of items that you may need to have on hand in order to complete this application online.
- List ALL Household Members (including yourself) whether they receive income or not. For household members receiving income, report each type of income and how often each source is received in whole dollars only. If they do not receive income from any source, choose no income.
- The school and grade of every student in your household. Although these are optional, it assists our office in processing the application for meal benefits.
- The last four (4) digits of the Primary Wage Earner or Adult Household Member Social Security Number who signs this form.
- If applicable, a valid Food Supplement Program (FSP) or Temporary Cash Assistance (TCA) case number for any member of your household.
