• The names, incomes and ages of every member of your household
• The school, grade, birthdate and student ID number of every student in your household
• Your social security number and electronic signature
• A valid email address and phone number for communication about the status of the application.

What you will need to know during the application process:

• Complete one application per household
• Parents are responsible for paying for all meal charges accrued before their meal application is approved
• Notification of meal eligibility will be received by US mail or email within 10 business days after applying
