For households who have both CEP and (Non-CEP) students, DO NOT FILL OUT THIS EDUCATIONAL BENEFITS FORM. Please fill out the Meal Benefit Application(clickable link here) for the household instead. The Educational Benefits Form (which you are currently on) will not provide the Non-CEP students with a meal benefit eligibility/status.

*If you are wanting to decline to apply for your CEP student and you are not seeking assistance for any of your students (CEP or Non-CEP/VLA), then you should continue with this form and follow the "Decline to Apply" instructions on the upcoming page.

WHAT YOU WILL NEED TO PROVIDE DURING THE ONLINE FORM PROCESS:

• The names and incomes of every member of your household (unless the household receives SNAP, KTAP, or FDPIR)
• The school, grade, birthdate of every student in your household (Optional)
• Your electronic signature
• A valid email address or phone number for the Child Nutrition Office to contact you regarding questions or the status of a form (Optional)

Before you begin...If you received a letter from the Fayette County Public Schools Child Nutrition Office saying that your children were automatically approved for the 2024-2025 school year because someone in your household participates in Directly Certified programs, then you do not need to submit a form. Please read the letter carefully to ensure all children in your household have been identified. If your household has been directly certified in previous school years but you have not received a letter prior to your child's first day at school, please go ahead and fill out the form.
