This Educational Benefits Form is only for students attending CEP schools in Fayette County Public Schools.
CEP Schools for the 2025–26 School Year:
Elementary: Academy of Millcreek, Arlington, Booker T. Washington, Breckinridge, Brenda Cowan, Cardinal Valley, Coventry Oak, Deep Springs, Dixie, Harrison, James Lane Allen, Julius Marks, Lansdowne, Liberty, Mary Todd, Meadowthorpe, Northern, Picadome, Russell Cave, Sandersville, Southern, Squires, Tates Creek, William Wells Brown, and Yates.
Middle: Bryan Station, Crawford, Leestown, Lexington, Morton, Southern, Tates Creek, and Winburn.
High: Bryan Station, Frederick Douglass, Henry Clay, and Tates Creek.
Other Programs: Audrey Grevious Center, Carter G. Woodson Academy, Family Care Center, Fayette County Preschool Center, George W. Carver STEM Academy for Boys, Martin Luther King Jr. Academy, Opportunity Middle College, Rise STEM Academy for Girls, Success Academy, The Learning Center, and The Stables.
If your household includes any students who do not attend CEP schools, do not complete this form.
Instead, please complete the Meal Benefit Application (clickable link here). The Educational Benefits Form you are currently on will not provide meal benefit eligibility for Non-CEP students.
Declining to Provide Income: If you are not requesting assistance for any students in your household—including CEP and Non-CEP—you may continue with this form and follow the “Decline Benefits” instructions on the next page.
What You Will Need to Complete This Form:
- Names and income details for every member of your household (unless receiving SNAP, KTAP, or FDPIR- you will need an active case number for these programs)
- The school name, grade, and birthdate of each student in your household (optional)
- Your electronic signature
- A valid email address or phone number for follow-up questions (optional)

Before You Begin: If you received a letter from the Child Nutrition Office stating that your children were automatically approved for the 2025–26 school year due to Direct Certification, you do not need to complete this form. Please review the letters to ensure all children in your household received one.
If your household was directly certified in the past but you have not yet received a letter this year, go ahead and complete the form to avoid delays, including all household members.
